As a member of the public service pension plan and to assist the Public Service Pension Centre in establishing future benefit payments for yourself and your survivors the following documents should be provided:

• Your Birth Certificate
• Your Spouse's Birth Certificate
• Your Children's Birth Certificate
• Your Children's Adoption Certificate
• Your Spouse's Death Certificate
• Your Marriage Certificate
• Divorce Certificate
• Separation Certificate
• Naming or Substitution of a Beneficiary form (PWGSC-TPSGC 2196)

As different Life Events occur throughout your career other documentation may be required

Please Contact Us - Public Service Pension Centre for other acceptable documentation that may be used as Secondary Evidence if you are unable to provide satisfactory birth or baptismal certificates.
