Plan Enrolment Form

Acknowledgement of plan membership is a mandatory requirement and as a plan member you are required to complete and sign the Enrolment Information and Acknowledgement of Plan Membership form (PWGSC-TPSGC 571) and return it to the Public Service Pension Centre.

Note: Complete this form only when requested by the Public Service Pension Centre. This form will be provided to you within your Plan Enrolment Package sent by the and should not be completed until that time.
