The selection boxes for Canadian Government Annuities. Beside the check box there is space in which to write your contract number.

The selection boxes for Veterans Affairs Canada are: Veterans Affairs Pension or Award; War Veterans Allowance or Veterans Affairs Financial Benefits. Beside the check box there is space in which to write your file number.

The selection boxes for Compensation are: Canadian Forces Pension; Public Service Pension; RCMP Pension and Judges’ Pension. Beside each check box there is space in which to write either your pension number

The selection boxes for Members of Parliament Retiring Allowances. Beside the check box there is space in which to write your personal record identifier.

Step 1: Instructions inform you to have your financial institution fill out this part of the form.
Step 2: Select one check box.

The choices are:

Bank Account: If you choose that option, you must fill in your bank account number; then your must indicate your Bank State Branch and account number;

Postal Account: If you choose that option, you must fill out the postal identification number and the account number

Building Society Account: If you choose that option, your must fill out the Bank State Brand and account number

And finally, the Bank Information Code (BIC/SWIFT), you must fill out the number.

Step 3: The Name of Account Holder;
Step 4: The Financial Institution’s Name, Address and Postal code;
Step 5: Signature of the Financial Institution
Step 6: The Financial Institution telephone number.

Then follow the text to authorize the Receiver General for Canada to deposit the payment (s) directly into your account until further notice.

Step 7: The boxes to fill out are for your signature and date.
