The selection boxes for the Canada Revenue Agency are: Income Tax Refund, GST/HST credit, WITB advance payments, and any other deemed overpayment of tax; Child Tax Benefit and Universal Child Care Benefit. Beside the check boxes there is a space in which to write your social number.

The selection boxes for Service Canada are: Old Age Security; Canada Pension Plan. Beside the check boxes there is a space in which to write your social number.

The selection boxes for Government Annuities. Beside the check boxes there is a space in which to write your contract number.

The selection boxes for Veterans Affairs Canada are: Veterans Affairs Pension or Award; War Veterans Allowance or Veterans Affairs Financial Benefits. Beside the check boxes there is a space in which to write your file number.

The selection box for National Defence and the Canadian Forces is: Canadian Forces Pension. Beside the check boxes there is a space in which to write your pension number.

The selection box for Public Service Pension Centre is: Public Service Pension. Beside the check boxes there is a space in which to write your file number.

In Step 6, it is stated: I, as the person entitled to receive the payment (s) indicated above, authorize the Receiver General for Canada to deposit the payment (s) directly into my account until further notice.
