The selection boxes for Veterans Affairs Canada are: Veterans Affairs Pension or Award; War Veterans Allowance or Veterans Affairs Financial Benefits. Beside the check box there is space in which to write your file number.
The selection boxes for Compensation are: Canadian Forces Pension; Public Service Pension; RCMP Pension and Judges’ Pension. Beside each check box there is space in which to write either your pension number

Step 1: Must be completed by the bank. The bank will be able to obtain the BRSTN number from the bank’s designated remittance processing unit location. Refer to the qualifying Financial Institutions list.
As well, mandatory Bank Routing Symbol Transit Number (BRSTN) is requested under this step. There is a box requesting the sort code number and a box for the account number
Step 2: Box is asking for the name(s) of the account holder(s);
Step 3: The Financial Institution’s Name, Address and Postal
code;
Step 4: The signature and date of an official at the Financial Institution;
Step 5: The Financial Institution telephone number.
Then follow the text to authorize the Receiver General for Canada to deposit the payment (s) directly into your account until further notice.
Step 6: The boxes to fill out are for your signature and date.
