Plan Members Compensation Advisors Public Service Pension Centre (PSPC) Regional Pay Office
Obtain general spousal and dependants benefits information from the "Your Public Service Pension and Benefits" Web portal or individualized information from the Public Service Pension Centre (PSPC). Provide a contact information sheet with the Pension and Benefits Web portal address, a toll free number, an email address, a facsimile and a mailing address for inquiries related to spousal and dependants benefits information, the completion of PWGSC-TPSGC 2196 Publiservice and designation of beneficiary. Provide all information about Supplementary Death Benefit (SDB), except for information on PWGSC-TPSGC 2196 Publiservice form or the naming of a beneficiary. Inform the plan member to forward all personal documents directly to the PSPC. Provide general and/or individualized spousal and dependants benefits information to members and general information to non-members. Provide a support function to help members and non-members navigate through the spousal and dependants benefits information on the Web site and provide explanations, if necessary.  
Forward personal documents directly to the PSPC. When processing other activities on the plan member's file, provide the PSPC with copies of personal documents from the file, if they have not previously been forwarded. Obtain or request personal documents from plan members, survivors, dependants or legal representatives.  
Obtain the "Naming or Substitution of a Beneficiary" form (PWGSC-TPSGC 2196 Publiservice) from the "Your Public Service Pension and Benefits" Web portal. If necessary request individualized Supplementary Death Benefit (SDB) designation of beneficiary information from the PSPC. Complete the PWGSC-TPSGC 2196 Publiservice and send it directly to the PSPC. Keep a copy of this form for personal records. Inform the plan members that they are to access the SDB designation of beneficiary form (PWGSC-TPSGC 2196 Publiservice) on the "Your Public Service Pension and Benefits" Web portal, to complete the form and forward it directly to the PSPC. No copies are to be retained by Compensation. Continue to provide plan members with SDB information (e.g. eligibility to contribute, arrears calculations). Continue to determine eligibility and deduct SDB contributions for clients serviced by the Regional Pay Office. Print the spousal and dependant benefits Web information including the SDB designation of beneficiary form (PWGSC-TPSGC 2196 Publiservice) and provide to individuals who do not have Internet access. Assist plan members with the completion of the SDB designation of beneficiary form (PWGSC-TPSGC 2196 Publiservice). Collect SDB contribution deficiencies.
In the event of death of a plan member, a representative of the member will provide the employer (manager, compensation advisor, etc.) with a copy of the death certificate. Provide the PSPC with a copy of the death certificate and contact information by completing and forwarding the PWGSC-TPSGC 2014. Certify salary, service and leave without pay (LWOP). Complete and submit the PWGSC-TPSGC 2386 form.

Forward the SDB claim form (PWGSC-TPSGC 2223), to the beneficiary for completion. Request certification of salary, service and leave without pay (LWOP).

Pay benefit.

 
  A) In the event of death of an active member with more than two years of pensionable service, provide the PSPC with contact information on form PWGSC- TPSGC 2008-1, and confirmation of eligible survivors. Initiate pension related payments.  
  B) For an active member with less than two years of pensionable service, complete and forward a copy of the PWGSC- TPSGC 2008 form to the PSPC. If the member did not have a survivor or dependant on the date of death, contact the PSPC for confirmation of the latest designation of beneficiary, if any. Once payee has been determined, complete appropriate section of PWGSC-TPSGC 2577 and forward to the Regional Pay Office. Compensation advisors from organizations not serviced by the Regional Pay Office must forward the PWGSC-TPSGC 2577 form to the PSPC. Pay return of contributions (ROC) for clients not serviced by the Regional Pay Office. Pay ROC for clients serviced by the Regional Pay Office. Complete appropriate section of PWGSC-TPSGC 2577 form and forward to the PSPC.
