SUBJECT: Public Service Health Care Plan Forms

1. PURPOSE

1.1 The purpose of this directive is to provide you with more information regarding the changes in processing Public Service Health Care Plan (PSHCP) insurance forms that will take effect on May 3, 2010.

1.2 It is important to note that a separate directive will be issued shortly regarding the new PSHCP Compensation Web Application. The new application will result in changes to PSHCP procedures.

2. BACKGROUND

2.1 This directive should be read in conjunction with Compensation Directive 2009-002 informing departments that compensation advisors would be solely responsible for the accuracy of all information -financial and non-financial - on insurance forms and in the Regional Pay System on April 1, 2010. This process has been postponed to May 3, 2010.

3. PROCEDURES/INSTRUCTIONS

3.1 Effective May 3, 2010, compensation advisors are to cease forwarding copies of the following forms to the Public Service Pension Centre:

  • the Public Service Health Care Plan - Employee Application (TBS -006491),
  • the Public Service Health Care Plan - Application for Designation of Additional Dependant for Comprehensive Coverage (PWGSC-TPSGC 2028-8 Publiservice),
  • the Notice of Cancellation - Public Service Health Care Plan (PWGSC-TPSGC 2397 Publiservice), and
  • any other written notification sent by the employee to cancel membership in the Plan.

These forms are to be retained on the employee's departmental Human Resources file.

3.2 Only the forms that must be completed when the employee terminates his employment, such as "Deductions from Annuity or Annual Allowance" form (PWGSC-TPSGC 1422) and the "Public Service Health Care Plan - Pensioner Application" (TBS -006492) as described in sections 2.10.1 and 2.11.1 of the Insurance Administration Manual (IAM) will continue to be sent to the Public Service Pension Centre.

3.3 All other procedures with respect to the administration of the PSHCP will continue to be followed as described in Chapter 2 of the IAM.
