Provision of the information requested on this document is voluntary. Provision of personal information is required pursuant to the Department of Public Works and Government Services Act, sections 7(1)(d) and 13 and will be used by the Pension Centre for the purpose of administrating the Public Service Superannuation Act (PSSA). This information is being collected for the purpose of documenting a result in loss of benefits and/or delays in processing incorrect pension estimates, benefits, or statements. Personal information is protected, and only used and disclosed in accordance with the Privacy Act and described in Personal Information Bank PWGSC PCE 702—Federal Pension Administration. Under the Act, individuals have the right to request access to their personal information and to request corrections, if erroneous or incomplete. To submit a request, individuals may contact the department’s Access to Information and Privacy Protection Directorate, Privacy Management Division at: tpsgc.vieprivee-privacy.pwgsc@tpsgc-pwgsc.gc.ca. At any time, individuals have the right to file a complaint with the Office of the Privacy Commissioner of Canada regarding PSPC’s handling of their personal information. In support of this request, I have attached a certified true copy of a court order or written agreement stating that I am to be named the beneficiary, or that I am entitled to this confirmation.
