How to Complete: Canada Revenue Agency Direct Transfer Form (T2151)

The Canada Revenue Agency Direct Transfer Form (T2151) records the transaction of moving funds from your public service pension plan to a locked-in registered retirement account of your choice. In this video, we will examine how to complete this form for a Transfer Value.

At the Government of Canada Pension Centre, we are here to help, and sometimes filling out forms can be tricky.

In this video, we will cover the Canada Revenue Agency T 2151 form.

We are going to walk you through the entire process to help you complete it easily. This form records the transaction of moving funds from your public service pension plan to a locked-in registered retirement account of your choice.

Before we move on, please note that we will show you how to complete the form for a Transfer Value Payment.

You can also use this form for other pension processes.

Complete section 1 with your personal information.

In the next portion of the form, Part A does not apply to you, so leave it blank.

(The form DIRECT TRANSFER OF A SINGLE AMOUNT UNDER SUBSECTION 147[19] OR SECTION 147.3 appears full screen. A male voice explains how to fill out the form. When the voice says: “Complete section 1…,” the personal information section becomes highlighted in yellow. When the voice says: “… Part A does not apply…,” the part A section becomes highlighted in yellow with a big red letter X on it.)

Check off the first box, I am a member of the RPP or Registered Pension Plan.

For the Canada Revenue Agency RPP Registration Number, enter 0288795.

This is our pension plan's registration number. For Employer's name, indicate Public service pension plan.

Next, for the Plan number, indicate your Pension number.

And finally for employers address, please indicate our mailing address Government of Canada Pension Centre Mail Facility, P.O. Box 8000 Matane, Quebec G4W 4T6.

(Male voice over. Part B—Transfer from an RPP is on screen. When the voice says “Check off the first box…,” the I AM A MEMBER OF THE RPP sentence has a checkmark appear in its box, and the sentence becomes highlighted in yellow. When the voice says: “For the Canada Revenue Agency…,” the RPP registration number blank field is populated with the numbers 0288795 and becomes highlighted in yellow. When the voice says: “For Employer’s name…,” the Employer’s name blank field is populated with the words Public Service Pension Plan and becomes highlighted in yellow. When the voice says: “… for the Plan Number…,” the Plan Number blank field is populated with the words Your Pension Number and becomes highlighted in yellow. When the voice says: “… indicate our mailing address…,” our mailing address appears in the Employer’s address blank field, and is highlighted in yellow.)

Let's move on. Check off the first box, transfer my whole entitlement under the plan identified in Part A or B.

This means that you want to transfer your entire pension entitlement.

(Male voice over. On the screen is PART C—DESCRIPTION OF THE AMOUNT TO BE TRANSFERRED. When the voice says: “Check off the first box…,” the first box of the two options is checked off, and that option becomes highlighted in yellow.)

On to the next section.

Check off the first box, transfer the RPP or A deferred profit sharing plan (DPSP) single amount to my registered retirement savings plan (RRSP).

When you check this box, you are indicating that you want to transfer your pension entitlement to your specified account. On the line next to the check box, this is where you add your RRSP plan number and name.

Note that the account number must be for your locked-in account.

The account number that you indicate on this form must match the account number that you specified on the Certification of the Lock-in Form, PWGSC-TPSGC 2347-18.

(Male voice over. On the screen is PART D—IDENTIFICATION OF THE RRSP, REGISTERED RETIREMENT INCOME FUND (RRIF), RPP, DPSP, SPECIFIED PENSION PLAN (SPP), or POOLED REGISTERED PENSION PLAN (PRPP) TO WHICH THE FUNDS ARE BEING TRANSFERRED options. When the voice says: “Check off the first box…,” the first option is checked and that option becomes highlighted in yellow. When the voice says: “…this is where you add your RRSP…,” the blank field has the words RRSP PLAN NUMBER AND NAME appear and becomes highlighted in yellow.)

The Employer's name and Canada Revenue Agency's registration number of RPP or DPSP are not applicable for this process.

Just leave them blank.

The Transferee's Address Box is where you indicate the address of the financial institution.

The address that you indicate on this form must match the address that you specified on the Certification of Lock-in Form, PWGSC-TPSGC 2347-18.

Next, please sign the Applicant's signature box. And, finally, indicate the current date in the appropriate box.

When the voice says: “The Employer’s name and the Canada Revenue Agency’s…,” these respective fields become highlighted in yellow and have a red X across them. When the voice says: “The transferee’s address box…,” that field is populated with the words FINANCIAL INSTITUTION’S MAILING ADDRESS and becomes highlighted in yellow. When the voice says: “Next, please sign…,” the Applicant’s signature field is populated with the words MEMBER SIGNATURE and becomes highlighted in yellow. When the voice says: “… indicate the current date …,” the year, month and day fields become highlighted in yellow.)

That's it. The only step left is to mail us your form here at the Pension Centre.

Please include your completed Certification of Lock-in for Purposes of the Public Service Superannuation Act or the Pension Benefits Division Act PWGSC-TPSGC 2347-18.
